
Twilight Physician Group is a non-profit primary care that was started in 2022 after noticing a major health gap in the older population. Our providers were tired of those 15 to 20 minute patient visits that obviously needed more time. They became providers to provide quality healthcare, not to provide a quantity of visits. Twilight Care, now known as Twilight Physician Group, started with a vision of going to the private residences of their patients to provide them with top-quality care that was also affordable for them and their families. And we aren't stopping there. We are making our goal to reach homes in all of New Mexico, no matter how rural or remote. Please follow us, as we continue to grow with our local communities.

Dr. Centenera is a geriatric medicine and hospice care physician with over 20 years of clinical experience. A graduate of Johns Hopkins University and the Robert Wood Johnson School of Medicine (Rutgers), he has served as Medical Director at multiple hospices over 12 years, consistently growing patient census to over 200 per program.
In addition to hospice, he practices as a Concierge Primary Care Physician, managing a panel of over 400 predominantly elderly patients who reside at home or in assisted living facilities.
Known for his clinical expertise and compassionate approach, Dr. Centenera is highly regarded and sought after by patients, families, and colleagues across the healthcare industry.

Sean is an integrative medicine clinician, educator, and healthcare executive with over 20 years of experience in primary care, functional medicine, East Asian medicine, and medical education. He is a board-certified Physician Assistant and licensed Doctor of Oriental Medicine in New Mexico, with advanced training in integrative psychiatry, nutritional psychiatry, personalized medicine, and ketamine-assisted therapy.
As Executive Director of Twilight Physician Group, he blends clinical practice with strategic leadership to advance patient-centered, whole-person care. His professional interests include chronic disease management, lifestyle and mind-body medicine, integrative mental health, and healthcare transformation.
A former Physician Assistant Program Director, Sean is passionate about interprofessional education and mentoring future clinicians. He is currently completing a Fellowship in Integrative Medicine through the University of Arizona's Andrew Weil Center.
His work focuses on integrating evidence-based medicine with compassionate, relationship-centered care to improve health for individuals and communities.

Erick Herrera is a clinical marketing specialist and healthcare professional in Albuquerque, NM, with over 10 years of experience in patient-centered care. His commitment to hospice and palliative care stems from personal experience caring for his grandparents at end of life.
Originally from El Paso, TX, he moved to Albuquerque at 15 and has since built his family, career, and community ties there. He began his healthcare training in Medical Assisting at Brookline College, gaining clinical experience in orthopedics and pain management before shifting focus to advocacy and outreach.
Under regional leadership, he advanced from Clinical Assistant to hospice marketing, spending nearly eight years connecting vulnerable patients and families with end-of-life services. Now expanding into primary care, he continues to grow daily, maximize resources for families, and support patients across all stages of care.

Christina Perez is the Office Manager at Twilight with over 20 years of progressive healthcare experience spanning frontline clinical roles to leadership positions. Her background includes primary care, specialty medicine, radiology, general surgery, and cardiovascular care—giving her a comprehensive understanding of medical operations across the continuum.
She holds a Bachelor of Science degree with a concentration in Hospital Administration and Human Resources, which supports her strengths in organizational management, workforce development, and regulatory compliance. Throughout her career, Christina has focused on improving workflows, enhancing patient experiences, and supporting providers in delivering high-quality, patient-centered care.
Known for her strategic mindset and results-driven approach, she is passionate about mentoring teams, building efficient systems, and fostering collaborative environments. Her mission is to bridge clinical and administrative excellence to improve outcomes for patients, providers, and the community.

Our Primary Care Providers have additional specialized training in treating elderly patients. They will do a comprehensive assessment and treatment to help patients maintain activities of daily living.

Twilight Physician Group uses the Montreal Cognitive Assessment (MoCA) test to evaluate people with memory loss or other symptoms of cognitive decline. They understand that patients with cognitive decline come with challenges. Part of their care is to be creative, flexible, and compassionate with their patients.

Our Providers assess the gait and balance of each of their patients. They are knowledgeable about the need for DME and CMS requirements. We also continue to build a lasting relationship with several physical, occupational, and speech therapists around the city of Albuquerque. Our optimal goal for each patient is to retain strength and movement.

Our Providers are highly educated on medication regimens, and it is a critical element in the comprehensive care of our patients. Our Providers work on non-adherence, adverse drug reactions, and preventative negative health outcomes related to medication.

Our Providers integrate their care with physical disease treatments and mood management. They look at the patient as a whole person that will need collaborative care between different health services such as physical therapy, medication management, and family support.

At TPG, we specialize in providing superb palliative care to our patients depending on their own specific needs. Our Palliative services are aimed at optimizing quality of life and mitigating suffering among people with serious, complex, and often terminal illnesses.

Twilight, Sol Amor Senior Care, & Luna Del Valle Hospice:
I have been working as a professional fiduciary since 2014 and over the years trying to find knowledgeable, caring, thorough, and responsive providers has always been a struggle. I have personally worked with just about every medical provider, home care agency, and hospice service in the greater Albuquerque area. My introduction to these organizations began through Twilight Physician Group and expanded thereafter following notice that they would be adding additional supportive services. Since finding these three organizations, I have been able to ensure that my clients are cared for by responsive and caring providers. Through their endeavors to expand services, they have demonstrated an ability to provide continuity of care through medical support, in-home caregiving services and, when appropriate, hospice care. In this post pandemic world, as organizations continue to struggle with staffing issues and employee turnover, Twilight Physician Group, Sol Amor Senior Care, and Luna Del Valle Hospice appear to have found reliability in their hiring practices.
